Joseph Doherty dc2df628e3 chore(followups): reviewer-recommended tests, comments, and hardening from the remediation reviews
The remediation reviews approved every task but left a tail of small notes.
This lands the gateway-side half of them.

Hardening (behavior changes, all narrow):

- BuildFilteredWriteBulkCommand's unreachable default case failed OPEN: a
  fifth bulk-write kind added upstream without a filter case here would have
  shipped the DENIED entries to the worker while reporting them denied to the
  caller. It now throws UnreachableException.
- SqliteCanonicalAuditStore.ListRecentAsync no longer throws on a row it
  cannot date. The retention sweep deliberately preserves such rows (SQLite's
  datetime() yields NULL, so the DELETE never matches), which guaranteed the
  dashboard's recent-audit view would meet one eventually and lose the whole
  page to it. The row is now reported at DateTimeOffset.MinValue with every
  other column intact, behind an optional logger.
- The audit drain loop's finally now completes the channel writer alongside
  detaching the drain, so a producer that raced past the attached check takes
  the write-through branch instead of stranding its event in a buffer nobody
  reads until shutdown. TryComplete is idempotent, so StopAsync is unaffected.

Tests:

- MapCommandReply ownership (Assert.Same on the inner reply), mirroring the
  existing MapEvent ownership test.
- Redactor key-id length boundary at exactly 64 and 65 characters, pinning
  which way it fails. Nothing validates key-id length at creation, so
  docs/Diagnostics.md's "which no issued key id does" is now stated as the
  heuristic it is.
- ApiKeyFailureLimiter.Reset with a PartitionResolution whose partition was
  evicted between the Check and the Reset: inert, and clears nobody else's
  block.
- Constraint-cache concurrency stress: the cap is enforced by the inserting
  thread, so overshoot must be transient and proportional to the in-flight
  inserters, and the cache must settle at or under the cap.
- ListRecentAsync against a raw-SQL undateable row.

Comment/doc accuracy:

- EventsHubViewerRegistry.ReleaseConnection records that it relies on
  SignalR's default sequential per-connection dispatch
  (MaximumParallelInvocationsPerClient = 1).
- A PERF(followup) note on Invoke's double session resolve and why removing it
  needs a SessionManager overload.
- SessionEventDistributor: the volatile-field comment named the pump as the
  lock-free reader, but the pump's single capture point is inside _replayLock;
  the genuinely lock-free reader is SubscriberCount. OnSubscriberOverflow's
  "cannot be observed here" now excepts the DisposeAsync abandon path. The
  churn test names its ConcurrentDictionary bucket-order assumption and that a
  violation surfaces as a read timeout, not a silent pass.
- The two "restores the sequential drain's behavior" claims (SessionManager,
  docs/Sessions.md) were wrong: the sequential drain leaked too, because
  KillWorkerAsync's entry ThrowIfCancellationRequested aborted the whole loop
  on the first session for zero kills. Reworded to "fixes a leak the
  sequential drain also had", with the sweep-bound/shutdown-unbound
  ParallelOptions asymmetry explained.
- ISessionManager.ShutdownAsync's token doc: it degrades the drain to a kill
  sweep rather than cancelling it, with the bounded overrun stated.
  SessionShutdownHostedService.StopAsync records that its cancellation-logging
  branch is now unreachable.

Joseph Doherty f920b4cbf5 fix(dashboard): clamped CAS retry loop in snapshot hub connection counter + direct counter tests
Decrement was decrement-first with a single non-retried repair CAS. From zero,
two unmatched decrements (SignalR calls OnDisconnectedAsync for a connection
whose OnConnectedAsync faulted) capture -1 and -2; a real Increment then makes
the count -1, and the first decrementer's stale CompareExchange(0, -1) matches
and resets to zero — erasing a live connection, so the idle gate freezes an open
dashboard. The same lost race also made Decrement report 0 when it had not
written 0.

Clamping now happens inside the compare-and-swap: read, clamp, publish, retry on
loss. A lost race re-reads the fresh value instead of repairing a stale one.

The counter moves to its own file per the one-public-type-per-file convention and
gains direct tests: the zero floor under concurrent unmatched decrements, matched
pairs settling at zero, and an interleaved connect/disconnect stress round. The
stress test asserts the observable invariants only — the specific interleaving
cannot be forced through the public API (verified: the previous implementation
passes it), which its remarks now state rather than implying a reproducer. A hub
wiring test is skipped for the EventsHub reason: driving Hub.OnConnectedAsync
needs caller-clients and connection-context fakes, and the overrides are two
lines of delegation to the tested type.

Also documents that the API-key refresh's pre-gate time check races benignly.

Joseph Doherty 77c5731b7b perf(dashboard): idle-gate the snapshot tick; cache static config; bound key-list refresh
The snapshot publisher broadcast to Clients.All on every ~1s tick forever, with
zero viewers. Each tick cost a session-registry snapshot and sort, a metrics
snapshot that copies dictionaries under the global metrics lock, a full rebuild
of EffectiveGatewayConfiguration, and a SQLite read of the API key table.

DashboardSnapshotHub now counts live connections into the singleton
DashboardSnapshotHubConnectionCounter (clamped at zero, since SignalR can call
OnDisconnectedAsync for a connection whose OnConnectedAsync faulted). The
publisher drives the snapshot enumerator by hand instead of await foreach: with
no connections it does not call MoveNextAsync at all, so the producing iterator
stays suspended at its yield and no snapshot is built — the gate removes the
build, not just the broadcast. It re-checks once a second, so the first viewer
resumes the tick within about one interval; that viewer is seeded immediately by
DashboardPageBase's synchronous GetSnapshot() and by the hub's OnConnectedAsync.

Two per-tick costs are bounded independently of the gate: the effective
configuration is startup-static (options are bound once at boot and never
reloaded), so it is built once and cached; and the API key summaries refresh at
most every 15s, since the list only changes when an operator creates, rotates,
or revokes a key. Only a successful refresh restarts the interval, so a failed
or timed-out read is still retried on the next tick with the previous summaries
left on screen.

Joseph Doherty 6c5218913b perf(dashboard): gate event mirror on live viewers — no clone, no send for unwatched sessions
DashboardEventBroadcaster.Publish ran a deep protobuf Clone (redaction is on by
default) and a group SendAsync for every event of every session, before anything
checked whether a dashboard client was actually watching. In the steady state the
session:{id} group is empty, so that work was thrown away per event.

SignalR does not expose group membership, so EventsHub now mirrors its own
add/remove into a singleton EventsHubViewerRegistry, and OnDisconnectedAsync
releases everything a dropped connection held (SessionDetailsPage disposes the
connection rather than unsubscribing). Publish returns early when the session has
no viewers, before the redaction clone. Watched sessions behave exactly as before.

Lazy mirror-lease start/stop was deliberately not attempted — it entangles the
dashboard with SessionEventDistributor subscribe lifetime for no saving beyond
this gate; recorded in docs/GatewayDashboardDesign.md.

Joseph Doherty e15c3cb052 fix(dashboard): redact mirror tag values when ShowTagValues off; trim value-log doc (SEC-25, SEC-30)
SEC-25 (near-term hardening; full per-session EventsHub ACL stays deferred to
roadmap item 12): DashboardEventBroadcaster now redacts tag values from a deep
CLONE of the event before mirroring to SignalR when Dashboard:ShowTagValues is
false (the default). Clears MxEvent.value (covers OnDataChange/OnWriteComplete/
OperationComplete/OnBufferedDataChange — their bodies are empty discriminators,
values ride in the top-level field incl. buffered arrays) and the alarm body's
current_value/limit_value. Source event never mutated (shared with the gRPC
path + replay ring) - verified by a source-not-mutated test. Makes the formerly
dead ShowTagValues flag live for the mirror. EventsHub TODO(per-session-acl)
kept and tied to roadmap item 12. Tests: DashboardEventBroadcasterTests (3).

SEC-30: trim docs/Diagnostics.md to mark opt-in command-value logging as NOT
YET IMPLEMENTED (no LogCommandValues knob, RedactCommandValue has no call
sites, no values logged); wiring deferred pending secured-bulk redaction
coverage (SEC-13). No option/call sites added.

Server build clean (0 warnings); Dashboard tests 152/152.

Joseph Doherty fca978de07 docs(src): add missing XML docs and strip tracking-ID comments
Sweep of 203 source files resolving CommentChecker findings: add
<summary>/<param>/<returns>/<inheritdoc> where missing, and remove
resolved task/issue tracking markers (Tests-NNN, Worker-NNN, Server-NNN,
Task N) from code comments. Comment/doc-only — no logic changes.
Server+Tests build clean under TreatWarningsAsErrors.

Joseph Doherty 2ead9bc200 fix(dashboard): close StartDashboardMirror/DisposeAsync race; internal-overflow test + metric label
(1) GatewaySession.StartDashboardMirror: publish _dashboardMirrorLease and _dashboardMirrorTask
    atomically under one _syncRoot section; if the session is already Closing/Closed/Faulted,
    dispose the just-created lease and return without starting the mirror task so nothing is orphaned.
(2) WaitUntilAsync test helper: catch OperationCanceledException and call Assert.Fail with the
    timeout duration and predicate source text instead of letting the exception propagate raw.
(3) New SessionEventDistributorTests.InternalSubscriberOverflow_HandlerSeesIsOnlySubscriberFalse:
    verifies CountExternalSubscribers excludes the internal subscriber, so isOnlySubscriber==false
    even when the internal subscriber is the only registered subscriber.
(4) SubscriberOverflowHandler delegate gains isInternal parameter; overflow metric label is
    "dashboard-mirror" for internal subscribers and "grpc-event-stream" for external ones.
(5) DashboardEventBroadcaster.Publish: wrap SendAsync Task acquisition in try/catch so a
    synchronous throw cannot escape the never-throw Publish interface contract.

Joseph Doherty 615b487a77 docs+ui: backfill XML doc comments and finish dashboard layout pass
Adds missing <summary>/<param> XML docs across 99 server, worker, and test
files so CommentChecker reports zero issues (TreatWarningsAsErrors needs the
analyzer clean). Bundles in WIP dashboard work: NavSection extraction,
MainLayout/site.css/js styling alignment, and DashboardOptions/Auth tweaks.

Joseph Doherty 327e9c5f94 Resolve Server-031..032 (re-triaged) + Server-038..043
Server-031: re-triaged. The recommended gateway-side
"skip-while-command-in-flight" guard is already in place at
WorkerClient.HeartbeatLoopAsync via WorkerClientOptions.HeartbeatStuckCeiling
(default 75s = 5× HeartbeatGrace). Two regression tests pin the
behaviour. Recommendation #1 (decouple worker-side _writeLock) is a
Worker-module concern (Worker-017 / Worker-023) and out of scope here.

Server-032: re-triaged. Recommendation #2 (rich diagnostic) is already
in EnqueueWorkerEventAsync, with #3 (overflow grace) absorbed by the
TryWrite → WriteAsync-with-timeout fall-through. Test
EnqueueWorkerEvent_WhenChannelFullPastTimeout_FaultsWithRichDiagnostic
pins the diagnostic string. Recommendation #1 (prose contract in
gateway.md / docs) is deferred — outside this pass's edit scope.

Server-038 (Security): EventsHub.SubscribeSession's missing per-session
ACL is documented with a TODO(per-session-acl) and a <remarks> block
explaining the v1 acceptance (any dashboard role can subscribe to any
session — non-secret metadata, redacted value logging). The per-session
ACL design lands in a follow-up once a session-scoped role exists.

Server-039 (Error handling): HubTokenService.Validate now rejects a
deserialized payload where both Name and NameIdentifier are null/empty.
New test file HubTokenServiceTests.cs covers the regression and five
sanity cases. TDD confirmed.

Server-040 (Conventions): MapGroupsToRoles gains a precedence comment
explaining "full literal match first, leading-RDN fallback;
OrdinalIgnoreCase via DashboardOptions.GroupToRole". Documentation-only.

Server-041 (Design adherence): EventStreamService.ProduceEventsAsync
wraps the broadcaster.Publish call in try/catch (Exception). The
producer loop and gRPC stream are no longer at the mercy of the
broadcaster's never-throw discipline. New regression test
StreamEventsAsync_WhenDashboardBroadcasterThrows_StillYieldsEventsAndDoesNotFaultSession.

Server-042 (Performance): DashboardSnapshotPublisher.ExecuteAsync now
mirrors AlarmsHubPublisher's reconnect loop — wraps the await foreach
in a while-not-cancelled, catches general exceptions, and Task.Delays
5s before retrying. An internal ctor accepts a shorter delay for the
test. New test file DashboardSnapshotPublisherTests.cs covers the
throw-then-yield reconnect path and the normal-completion case.

Server-043 (Documentation): HubTokenService class XML doc gains a
<remarks> describing the singleton lifetime, the two consumer scopes
(DashboardHubConnectionFactory scoped, HubTokenAuthenticationHandler
transient), and the thread-safety contract.

Verification: dotnet build src/ZB.MOM.WW.MxGateway.slnx clean
(0 warnings / 0 errors); src/ZB.MOM.WW.MxGateway.Tests 486/486 passing.

Joseph Doherty d692232191 dashboard: clear deferred items — EventsHub publisher + doc refresh
EventsHub publisher (closes the v2.1 follow-up flagged in the previous commit)

EventStreamService now mirrors every MxEvent it forwards to a gRPC client
into the `EventsHub` group for the session. The fan-out goes through a new
singleton `IDashboardEventBroadcaster`:

  * IDashboardEventBroadcaster — abstraction so EventStreamService doesn't
    take a direct dependency on SignalR.
  * DashboardEventBroadcaster — singleton implementation that hands the
    SendAsync to IHubContext<EventsHub> as fire-and-forget. Errors are
    logged at debug and dropped so the source gRPC stream is never
    blocked.

EventStreamService now takes IDashboardEventBroadcaster as a ctor parameter
and calls Publish(sessionId, publicEvent) once per event after sequence
filtering, before the bounded queue write. Test fixtures and the live
integration harness pass NullDashboardEventBroadcaster.Instance so the
broadcaster is a no-op in unit tests.

SessionDetailsPage adds a "Recent events" panel:
  * implements IAsyncDisposable
  * opens a second HubConnection via DashboardHubConnectionFactory targeting
    /hubs/events
  * calls SubscribeSession(SessionId) on Start
  * renders the most recent 50 events in a small table (worker seq, family,
    server/item handle, alarm reference when the event is OnAlarmTransition)
  * shows a live/offline conn-pill driven by HubConnection.Closed /
    Reconnected events

The dashboard mirror is intentionally passive — events appear only while a
gRPC client is also consuming that session's events. Documented as such in
the empty-state copy and in GatewayDashboardDesign.md.

Documentation refresh

Every doc that referenced the retired options (PathBase, RequireAdminScope,
RequiredGroup) and the old API-key-cookie auth flow is updated to describe
the new model:

  * CLAUDE.md — Authentication section now explains LDAP bind +
    GroupToRole + HubToken bearer flow.
  * gateway.md — Dashboard section: root-mounted routes, snapshot/alarms/
    events SignalR hubs, LDAP cookie + bearer scheme.
  * docs/GatewayConfiguration.md — drop PathBase / RequireAdminScope rows,
    add GroupToRole row, append "Authorization policies" and "SignalR hubs"
    subsections describing the three policies and the /hubs/* endpoints.
  * docs/GatewayDashboardDesign.md — hosting model (root mount, new
    endpoint layout), Realtime Updates rewritten as a hub table
    (DashboardSnapshotHub / AlarmsHub / EventsHub with producers, payloads,
    and routing), Authentication And Authorization rewritten around LDAP +
    role mapping + the hub bearer flow, Configuration block updated.
  * docs/GatewayProcessDesign.md — security-section dashboard paragraph
    and the example config block both refreshed to LDAP/role auth.
  * docs/ImplementationPlanGateway.md — dashboard-auth deliverable list
    updated (LDAP bind + GroupToRole + /hubs/token bearer mint replace the
    API-key login flow).
  * docs/GatewayTesting.md — DashboardLdapLiveTests blurb describes the
    GroupToRole fixture (`{ GwAdmin: Admin }`) instead of the retired
    RequiredGroup default; success-path assertion explains the role-claim
    check.

Verification: 475 server tests, 275 worker tests (+ 9 dev-rig skips), 18
integration tests (live MxAccess + LDAP + Galaxy) all pass — including the
live worker smoke test fixture that now constructs EventStreamService with
the new broadcaster parameter.

Joseph Doherty 65943597d4 dashboard: side-rail layout + SignalR push hubs (snapshot, alarms, events)
Layout
------
DashboardLayout.razor replaces the inline header nav with a left side rail
modelled on the OtOpcUa admin (Dashboard B). The top bar keeps only the
brand, breadcrumb, and signed-in status pill; navigation moves into a
fixed-width 218px rail with grouped section eyebrows (Overview,
Runtime, Galaxy, Admin) and a Session footer carrying the user name,
role claims, and a Sign-out button. dashboard.css gains the
`.app-shell` flex container, `.side-rail` column, `.rail-eyebrow`,
`.rail-link[.active]`, `.rail-foot`, `.rail-user`, `.rail-roles`, and
`.rail-btn` rules (all driven by the existing theme.css tokens, no new
hard-coded colours).

SignalR (push)
--------------
Adds three hubs under `Dashboard/Hubs/`, all gated by the
`HubClientsPolicy` registered in the previous commit:

  * DashboardSnapshotHub (/hubs/snapshot)
    Broadcasts the full DashboardSnapshot on every change. Sends the
    current snapshot to a new caller in OnConnectedAsync so the first
    paint is immediate.

  * AlarmsHub (/hubs/alarms)
    Connected clients auto-join the `__alarms__` group. Receives
    AlarmFeedMessage values (active_alarm / snapshot_complete /
    transition) re-broadcast from the gateway's central alarm monitor.

  * EventsHub (/hubs/events)
    Per-session push surface. Clients call SubscribeSession(sessionId)
    to join `session:{id}`. The publisher side is intentionally a
    follow-up — the snapshot hub already carries recent-events
    rollups; a dedicated MxEvent broadcaster on EventStreamService
    will plug into this hub's group convention.

Two BackgroundService publishers wire server-side data sources to the
hubs:

  * DashboardSnapshotPublisher subscribes to
    `IDashboardSnapshotService.WatchSnapshotsAsync` and forwards every
    snapshot to all connected hub clients.
  * AlarmsHubPublisher subscribes to `IGatewayAlarmService.StreamAsync`
    (no filter) and forwards every AlarmFeedMessage to the
    `__alarms__` group, reconnecting with a 5-second backoff if the
    stream faults.

Connection + auth plumbing
--------------------------
  * `GET /hubs/token` issues a fresh data-protected bearer token
    bound to the calling user's identity and roles. Gated by the
    cookie-only ViewerPolicy so a Blazor circuit (cookie-authenticated)
    can mint a token, but a hub bearer cannot self-bootstrap a new
    one.
  * DashboardHubConnectionFactory (scoped) is the client-side helper
    Razor pages inject. It builds a HubConnection with an
    AccessTokenProvider that calls HubTokenService.Issue on every
    (re)connect — keeps the connection alive across cookie refresh
    boundaries.

Pull → push refactor
--------------------
DashboardPageBase no longer drives its own `WatchSnapshotsAsync`
async-foreach loop. It now:
  1. seeds Snapshot synchronously from `IDashboardSnapshotService.GetSnapshot()`
     so the first render is non-empty;
  2. opens a `DashboardSnapshotHub` connection via the connection
     factory;
  3. updates Snapshot + triggers StateHasChanged on each
     `SnapshotUpdated` push.

The hub connection is best-effort: if SignalR can't start, the
synchronous snapshot seed keeps the UI populated. SignalR's
WithAutomaticReconnect handles the recovery path.

Package
-------
Adds `Microsoft.AspNetCore.SignalR.Client` 10.0.0 to the server csproj
so the in-process Blazor pages can open hub connections back to their
own hosting process.

Verification: 475 server tests (+ 2 new
`DashboardHubsRegistrationTests` that pin the hub negotiate endpoints
and the singleton/scoped DI shape), 275 worker tests (+ 9 dev-rig
skips), 18 integration tests (live MxAccess + LDAP + Galaxy) all pass.
