Defect: the hourly KPI rollup fold (KpiHistoryRepository.FoldHourlyRollupsAsync)
sums every Rate-classified sample recorded in an hour, correct only if each
sample is a non-overlapping delta covering exactly one recorder tick
(KpiHistoryOptions.SampleInterval). That invariant was an unenforced silent
assumption across three independently-owned config keys — SampleInterval,
NotificationOutboxOptions.DeliveredKpiWindow, SiteCallAuditOptions.KpiInterval —
so widening DeliveredKpiWindow to e.g. 15 min would silently over-count the
rollup ~15x.
Robust fix: NotificationOutboxKpiSampleSource and SiteCallAuditKpiSampleSource
now rescale their Rate-classified samples (deliveredLastInterval,
failedLastInterval) by SampleInterval / (DeliveredKpiWindow or KpiInterval)
before recording, so the rollup's per-hour Sum is correct regardless of how
the other two keys are tuned — the live dashboard tile's own cutoff/window is
untouched. Both sources gain a project reference to KpiHistory to read
SampleInterval; no cycle (KpiHistory references only Commons).
Eager cross-validation (defense-in-depth): KpiRateFoldIntervalCrossValidator
(Host, the one composition root referencing all three component projects)
fails boot naming all three config keys when they diverge, protecting the
separate (still-real) invariant that the recorded/charted KPI-history sample
equals the live tile at the same instant — which the rescale trades away
whenever the scale factor isn't 1. Rides KpiHistoryOptions' existing
ValidateOnStart trigger.
Also: NotificationOutboxOptions had no registered validator at all before this
(the gap this task's "whichever options class owns DeliveredKpiWindow" file
scope pointed at) — added NotificationOutboxOptionsValidator mirroring
SiteCallAuditOptionsValidator's positive-duration/count convention, wired with
ValidateOnStart.
Tests: new rescale tests in both sample-source suites prove the fold is
numerically correct at a non-default SampleInterval (30/40 counted over a
15/20-minute window correctly divides down to a 1-minute-tick delta); new
KpiRateFoldIntervalCrossValidatorTests prove divergent config fails with all
three keys named; new NotificationOutboxOptionsValidatorTests. Every new test
proven to fail against the unfixed computation first (temporarily
short-circuited the scale factor / cross-check to the pre-fix behavior,
confirmed failure, restored) — not via git stash (repo-wide, shared across
concurrent worktrees this session) but via file-copy swap.
Full suites green: Host.Tests (527), ConfigurationDatabase.Tests (401),
KpiHistory.Tests (54), NotificationOutbox.Tests (163),
SiteCallAudit.Tests (67).
Register row 45. NotificationOutboxActor.ResolveAdapters() cached adapter
instances in an actor-lifetime IServiceScope (_adaptersScope), but both
EmailNotificationDeliveryAdapter and SmsNotificationDeliveryAdapter are
AddScoped and hold a scoped INotificationRepository over one EF DbContext.
MaxParallelDeliveries (default 4) then drove up to four concurrent deliveries
through that single adapter and single context, which central-b logged
continuously on the rig as "InvalidOperationException: A second operation was
started on this context instance before a previous operation completed". The
collided delivery's row is never written and the wasted attempt counts against
its retry budget, so sustained contention could park a legitimate notification.
The per-delivery scope DeliverGatedAsync already opened for the outbox
repository — added for exactly this hazard — did not cover the adapter.
_adaptersCache, _adaptersScope and ResolveAdapters() are deleted. Delivery
adapters are now resolved inside the per-delivery scope via a static
ResolveAdapter(IServiceProvider, NotificationType), preserving DI's last-wins
resolution semantics (the enumeration runs to completion rather than
short-circuiting) and the null-adapter park path. Nothing shared is lost: the
Email adapter builds a fresh SMTP client per attempt from an injected factory
and the SMS adapter draws pooled connections from the singleton
IHttpClientFactory, so no adapter carried state worth preserving across
deliveries. The Central UI's NotificationChannelCatalog is an independent
consumer of the same scoped registrations and is untouched, so the
adapter-gated Type selector still enumerates exactly the deliverable channels.
Tests: Dispatch_ResolvesAdaptersOnce_AcrossMultipleSweeps (which pinned the
superseded caching behaviour) is replaced by
ParallelDeliveries_EachGetTheirOwnAdapterAndDbContext — two parallel deliveries
through a scoped repository whose simulated DbContext throws EF's exact
second-operation exception on overlap, with a rendezvous that guarantees the
overlap window — and Dispatch_ResolvesAdapterPerDelivery_NotOncePerActorLifetime.
Both fail against the pre-fix actor (stash-verified: 1 of 2 rows persisted; 1
distinct adapter instead of 4) and pass after. Parallel-delivery
ordering/throughput semantics are unchanged.
Docs: Component-NotificationOutbox.md gains the scoped-adapter contract under
Delivery Adapters (resolve per delivery; adapters carry no cross-delivery
state; genuinely shared resources belong in a singleton/factory), and the
NotificationOutbox-006 finding is marked SUPERSEDED with the reasoning.
Solution build clean; NotificationOutbox (153), AuditLog (383), integration
NotificationOutbox flow (2) and CentralUI notification (14) suites green.
Remove project bookkeeping citations from shipped code comments across the
solution: hyphenated task IDs (WP-14, StoreAndForward-025), milestone/task/
issue refs (M3, Task 4, Audit Log #23, #21), Bundle X task-bundle labels,
and C/D/K/S/T phase labels.
Comment text only — no code logic, string/log literals, or XML-doc structure
changed. Genuine descriptions are preserved (only the citation is stripped),
and technical lookalikes are retained (UTF-8, SHA-256, T00:00:00, M365,
UTC-5, pre-C4/pre-C5 schema versions). Flagged by the new CommentChecker
TaskReferenceInComment / TrackingReferenceInComment checks plus targeted
grep passes; full solution builds clean, append-only guard tests pass.
Resolve all 622 issues flagged by the enhanced CommentChecker: add missing

add missing <summary> tags, and replace misused/redundant <inheritdoc/> on

Documentation-only — no behavior change; solution builds clean.
