Joseph Doherty fd5e023d08 fix(comms): review findings — consumer-based debug orphan net, foreign-cancel triad, honest onConnected, served-row-exact retirement, full-rate reconcile
F1 (HIGH) DebugStreamBridgeActor: the 5-minute orphan net measured the MAILBOX
(SetReceiveTimeout), and once stream events were correctly marked
INotInfluenceReceiveTimeout nothing recurring reset it — the snapshot lands once
and GrpcStreamStable once — so every healthy session self-terminated at ~6 min
with a false "Site disconnected". Replaced with a periodic self-tick
(ConsumerLivenessCheckInterval, 30s) over a consumer-last-seen stamp renewed only
by DebugStreamConsumerAlive, which DebugStreamService Tells on a shared timer to
every session still in its registry (holding a session there IS "a consumer is
attached" — both the Blazor view and the SignalR hub release it on
dispose/disconnect, and it works headless). Reverting the wrapper was rejected: it
would restore the quiet-instance orphan bug.

F2 (MED) SiteStreamGrpcClient: the RpcException(Cancelled) filter now requires
cts.IsCancellationRequested. A peer-originated / channel-dispose Cancelled fired
none of onError/onCompleted/onConnected, leaving SiteAlarmAggregatorActor with
_streamDown=false forever (IsLive stuck true, reconcile reopen guard never fired).

F3 (MED) SiteStreamGrpcClient: a header TIMEOUT is no longer reported as
connected — that shape is exactly what an unreachable site produces, and it
cleared _streamDown, consumed _seedOnConnect and launched a full snapshot fan-out
at a dead site. AwaitHeadersAsync returns bool; the first received event is the
fallback connected signal, fired at most once from headers OR first event.

F4 (LOW-MED) SqliteAuditWriter.MarkReconciledUpToAsync: the blanket below-cursor
UPDATE retired late-stamped inserts that were never served (then age-purged —
silent loss). The flip is now bounded by insertion order: a Pending row retires
only if its rowid is at or below the high-water mark of rows this instance has
served from ReadPendingSinceAsync (clamped on purge, since SQLite reuses rowids);
Forwarded rows are exempt (central ACKed them over the push path). At-least-once
is unchanged.

F5 (LOW) Documented the liveness dependency (a served row never covered by a later
cursor stays Pending forever; PurgeExpiredAsync never purges Pending) in
ISiteAuditQueue + Component-AuditLog.md, and added a cheap site-health signal:
SiteAuditBacklogReporter logs a rate-limited warning when the existing
oldest-pending metric exceeds 24h.

F6 (MED) SiteAlarmAggregatorActor: _fanoutSinceLastTick was armed by the
reconcile's OWN fan-out, so steady state ran fan-out→skip→fan-out→skip — one
reconcile per 2x interval (120s), halving the not-reporting refresh and the alarm
reconcile backstop. The skip is now armed only by connect/failover-driven seeds
(initial, _seedOnConnect, and a re-seed queued behind one).

Tests: Communication.Tests 691 passed (+13), AuditLog.Tests 382 passed (+5).

Joseph Doherty 5d075f1374 fix(central): review findings — no client-side audit truncation, insert-first upsert, QI-safe scripts, honest operator replies
Six adversarial-review findings in the central SQL/ingest layer.

F1 (AuditLogRepository.InsertChunkAsync) — the set-based ingest declared each
string parameter at its COLUMN width (Actor/Target 256, Action 64, Outcome 16,
Category 32, SourceNode 64), so SqlClient truncated an over-long value at bind
time and committed the mutilated row — silent, in an append-only store, with no
PayloadTruncated flag — while the per-row and reconciliation paths sent the same
value in full and let the server reject it with 2628. Bind at the value's own
length instead; explicit SqlDbType is kept (it fixes the VALUES constructor's
derived column types and datetime2 precision). Design: reject everywhere,
truncate nowhere — matching today's per-row behaviour.

F2 (SiteCallAuditRepository.UpsertAsync) — the single-statement upsert ran the
monotonic UPDATE first and INSERTed only if nothing matched. Two writers racing
the first packet of one TrackedOperationId (the cached dual-write and the
reconciliation pull carry DIFFERENT lifecycle states) both matched nothing, and
the loser then skipped its INSERT or swallowed a 2627 — dropping its
Status/RetryCount/HttpStatus/TerminalAtUtc. Legs swapped to
`IF NOT EXISTS … INSERT; UPDATE <monotonic>` — still one round trip, and the
loser's UPDATE now lands on the winner's row. The duplicate-key catch re-runs
the monotonic UPDATE for the same reason. Moved to raw SQL with explicitly-typed
parameters so the intricate rank predicate exists in exactly one place (an
untyped DateTime would bind as `datetime` and round the freshness tiebreaker).

F3 (docs/plans/sql/*.sql) — filtered-index DDL failed with error 1934 under the
documented `docker exec … sqlcmd` path, which defaults QUOTED_IDENTIFIER OFF;
once IX_Notifications_Delivered exists, QI-OFF DML on Notifications fails too.
All four scripts now open with `SET QUOTED_IDENTIFIER ON; SET ANSI_NULLS ON; GO`
(own batch, so it is in force when the next batch parses), and the migration
convention in Component-ConfigurationDatabase.md documents `sqlcmd -I`. Verified
live: the pre-fix script fails 1934 without -I, the fixed one applies.

F4 (SiteCallAuditActor) — the off-mailbox reconciliation/purge passes reuse the
injected repository, so tests drove one DbContext from the pass and a mailbox
handler concurrently. Serialized at the CALL via a private SerializedRepository
wrapper applied only by the test constructors, rather than running the pass
on-mailbox: production keeps its PipeTo shape untouched, and the existing
"a blocked drain does not stall ingest/query/KPI" regression tests stay
meaningful (they would have been invalidated by suspending the mailbox).

F5 (AuditLogIngestActor) — when the batch failed because the 20 s IngestBudget
expired, the per-row fallback reused the same expired token: N instant failures,
N counter bumps, zero accepted. The fallback now gets a fresh 5 s budget (inside
the 30 s outer Ask), and a blown budget bumps the failure counter ONCE for the
batch instead of once per row.

F6 (NotificationOutboxRepository.UpdateAsync) — ExecuteUpdate's row count was
discarded, so an operator Retry/Discard of a notification the retention purge had
already deleted reported success (the pre-ExecuteUpdate code threw
DbUpdateConcurrencyException). UpdateAsync now returns whether a row matched; the
operator one-shots answer "notification not found" and emit no audit row for the
action that did not happen, while the dispatcher logs a warning (its delivery
already happened; nothing to retry). GetByIdAsync switched to AsNoTracking since
the write is out-of-band.

Tests: 5 new SQL-backed regressions (over-long Target rejected on both paths +
boundary round-trip; concurrent first-write and already-created-by-another-writer
upserts; vanished-row UpdateAsync), a token-identity pin on the ingest fallback,
a repository-concurrency detector for the SiteCallAudit passes, and vanished-row
operator-path tests. The F1/F2/F4 regressions were each confirmed failing against
the pre-fix code. Suites: ConfigurationDatabase 369, AuditLog 378, SiteCallAudit
66, NotificationOutbox 152 — all green, solution builds with 0 warnings.

Joseph Doherty cfa6acbf48 fix(test): order the cached-drain MarkForwarded assertions behind the push they follow
CachedDrain_OrphanRow_PastGrace_IsAbandoned_AndTheValidRowStillFlows gated on
IngestCachedTelemetryAsync being received once and then asserted, bare, that the
valid row had been marked Forwarded. The drain does that strictly AFTER the push
returns: OnCachedDrainAsync abandons the orphan (:351), pushes the batch (:366),
then parses the ack and marks the accepted ids (:380). Observing the push
therefore orders nothing with respect to the second MarkForwardedAsync — under a
loaded parallel run the post-push continuation can be scheduled after the poll
that saw the push, and the assertion fails fast with "Actually received no
matching calls" while the orphan's own earlier call is reported as the single
non-matching one.

Reproduced deterministically by delaying only the post-push step, which fails
exactly this test (11 siblings still pass) at ~1.3s into the assembly run —
matching the observed failure's fast-fail signature and pointing at line 430.
With the fix the same injected delay passes; suppressing the valid row's
MarkForwarded entirely still fails the test with the identical message, so the
claim (orphan abandoned in its own call, valid row pushed and marked, exactly
once each with exactly the same arguments) is unchanged in force.

Same unsynchronized-assertion class as c4caebe9, different actor. Test-only; the
drain's abandon/push/mark ordering is correct as written.

Joseph Doherty c4caebe9b4 fix(test): remove the unsynchronized audit-attempt assertion in the two dispatcher audit-safety tests
Both NotifyDispatcher_AuditWriter_Throws_DeliveryStillSucceeds and
NotificationDispatch_BrokenAuditWriter_StillTransitionsToDelivered read the
throwing writer's attempt counter with a bare Assert immediately after an
AwaitAssert on the Notifications row reaching Delivered. That assumes the audit
writes happen no later than the operational status write, which the dispatcher
deliberately does NOT guarantee: DeliverOneAsync persists the delivery state
first (NotificationOutboxActor.cs:657) and only then emits the Attempted
(:663) and terminal (:676) audit rows — audit is best-effort and must never
gate the user-facing action. Observing Delivered therefore establishes no
happens-before edge with the writer, and under a loaded full-solution parallel
run the continuation after the DB write can be scheduled after the poll that
saw Delivered, so the counter reads 0 and the test fails with "saw 0".

Reproduced deterministically by delaying only the post-update audit emission,
which yields both observed failure messages verbatim; with the fix in place the
same injected delay passes, and suppressing the emissions entirely still fails
both tests with the identical messages — the claims (delivery despite audit
failure, and attempts >= N) are unchanged in force, only the ordering
assumption is gone.

Test-only change; the update-then-audit ordering predates the remediation
(#23 M4) and is correct as written.

Joseph Doherty 2e4e41a8f7 fix(auditlog): site audit DB onto the data volume; required path + soft flush
Closes WP1.2 of the arch-review remediation plan (finding #2, High):
SqliteAuditWriterOptions.DatabasePath defaulted to CWD-relative "auditlog.db",
which on the docker rig resolves onto the container's ephemeral overlayfs
(not the mounted /app/data volume), silently discarding the pending audit
forward-state backlog on every recreate; nothing in docker/ or docker-env2/
overrode it; FlushIntervalMs was validated but never read by the writer loop
(one commit per event even at trickle rate); and no PRAGMA synchronous was
set (SQLite's FULL default fsyncs every commit).

- DatabasePath now has no default (mirrors ZB.MOM.WW.LocalDb's LocalDbOptions.Path)
  and is required pre-host for Site nodes only, via a new StartupValidator raw-config
  check (top-level "AuditLog:SiteWriter:DatabasePath", NOT nested under ScadaBridge:
  AddAuditLog binds that section off the configuration root). SqliteAuditWriterOptionsValidator
  deliberately does NOT check DatabasePath itself, because AddAuditLog runs its
  ValidateOnStart on both Central and Site composition roots but only Site nodes
  ever resolve the writer — checking it there would fail Central's boot too.
- All 8 site-node appsettings under docker/ and docker-env2/ now set
  AuditLog:SiteWriter:DatabasePath to /app/data/auditlog.db (mounted volume,
  survives container recreate, same convention as LocalDb:Path); the local-dev
  base appsettings.Site.json sets ./data/auditlog.db to match.
- The writer loop now honors FlushIntervalMs: after draining the immediately
  available burst, it keeps the transaction open (bounded by FlushIntervalMs
  from the first event) waiting for more trickle-rate events before committing,
  instead of flushing (and fsyncing) per event.
- PRAGMA synchronous = NORMAL on the write connection — audit is best-effort by
  design (CLAUDE.md: "Audit-write failure NEVER aborts the user-facing action"),
  so NORMAL's narrower power-loss window is an acceptable trade for far fewer
  fsyncs; WAL mode still guarantees no corruption.
- Tests: StartupValidator site-required/blank/central-exempt cases; writer
  trickle-load single-transaction coalescing + beyond-interval separate-transaction
  regression (new FlushCountForTests seam); options-validator doc updates reflecting
  the moved responsibility. Full suite runs green: AuditLog.Tests 368/368,
  Host.Tests 480/480.

One-time migration note: the existing container-local auditlog.db (wherever it
landed under CWD) is abandoned by this change, not migrated — already-forwarded
rows are safe centrally (AuditLog is the durable copy), and any still-Pending
rows on the abandoned path are lost once. This is the exact bug being fixed, not
a new loss: those rows were already living outside the mounted volume and would
not have survived the next container recreate regardless. Cross-reference
docs/known-issues/2026-07-20-cached-telemetry-drain-hot-loop.md, which this
placement bug caused.

Joseph Doherty 006202f3c7 fix(audit): fail closed when a configured redactor is unavailable (#35)
Component-AuditLog.md has always required "we over-redact, never under-redact,
on configuration faults", but the body / SQL-parameter redactors violated it.

AuditRegexCache rejects a pattern that is malformed OR whose compile exceeds a
100 ms budget, caching the rejection for the process lifetime.
ScadaBridgeAuditRedactor then simply dropped the rejected pattern from its
redactor set and emitted the payload anyway — publishing precisely the values
the operator configured it to suppress, onto a row that looks entirely normal
downstream. Recovery required a process restart and the only signal was one
Warning line. The SQL path was worse: TryGetSqlParamRedactor returned a bare
false for both "no redactor configured for this connection" and "the configured
one will not compile", and CLAUDE.md records SQL parameter capture as on by
default.

Two changes:

1. Fail closed. A pattern that is CONFIGURED but unavailable now over-redacts
   the whole payload and increments AuditRedactionFailure, reusing the existing
   safety-net path. "Not configured at all" stays permissive — conflating those
   two states is the actual defect, so both are pinned by tests.

2. Precompile off the hot path. The audit-log roadmap specifies patterns are
   "precompiled at startup; rejected if compile takes >100ms"; the implementation
   had drifted to compiling lazily on first event, which put a wall-clock budget
   on a hot path under production load. RegexOptions.Compiled emits IL during
   construction, so a busy node could blow the budget on a perfectly valid
   pattern. Warm-up now runs at construction and on every options reload. The
   residual window between a reload and its warm-up is safe because that path
   now fails closed.

Warm-up deliberately does not fail the boot — an unusable pattern degrades the
node to over-redaction (safe, loud) rather than refusing to start. Reading
CurrentValue happens inside the warm-up try so an options provider that throws
still surfaces via Apply's over-redact path, not the constructor
(OuterCatch_OptionsThrows_NeverLeaks_AllSensitiveFieldsOverRedacted).

Also de-flakes GrpcCentralTransportTests.DeadlineExceeded_IsNotRetriedOnThePeer,
which is how this was found. It black-holed node A behind a 300 ms deadline, but
on a saturated machine the call could fail to even START — a genuinely-unsent
failure that IsConnectFailure correctly fails over on, so node B's ack arrived
instead of the expected Status.Failure. The test read as a flake while actually
reporting that its own premise had not held. Split in two: the hard rule now
injects an explicit DeadlineExceeded via a trailers-only response (deterministic,
load-independent), and a new BlackHoledNode_DoesNotHang covers the
deadline-is-actually-applied half with both nodes black-holed so no ack can
arrive down any path.

Verified: both fixes were confirmed to fail before they pass — reverting the
fail-closed guard fails exactly the 5 fail-closed tests while the 4 controls
still pass, and adding DeadlineExceeded to IsConnectFailure fails the rewritten
transport test. AuditLog 367/367, Host.Tests GrpcCentralTransport 8/8, solution
build clean. The previously-intermittent
Filter_PicksUp_NewBodyRedactor_OnConfigReload is green in a full sweep for the
first time.

Not addressed here, and noted on #35: the 100 ms wall-clock budget remains a
weak proxy for catastrophic backtracking (RegexOptions.Compiled defers JIT to
first match, so construction time measures the wrong thing), and a rejection is
still cached permanently. Both are now safe rather than dangerous, so they are
hardening rather than a leak.

Joseph Doherty 63c16d6912 refactor(comm): retire ClusterClient naming after the gRPC cutover
Phase 4 of the ClusterClient -> gRPC migration deleted the Akka transports
but left the naming behind: `ClusterClientSiteAuditClient` was transport-
agnostic and worked unchanged, so it survived the deletion under a name
that now describes a transport the repo no longer has. Same for a scatter
of doc-comments still framing gRPC as "the new transport" beside an Akka
one that is gone.

Renames it to `SiteCommunicationAuditClient` (and its test file) and
rewrites the stale comments to describe the single transport that exists.
Also tightens CLAUDE.md: drops the self-describing directory listing and
the 27-component enumeration in favour of the non-obvious parts only.

Behaviour-neutral: names and prose only. Recorded as the Phase 4
follow-up in docs/plans/2026-07-22-clusterclient-to-grpc-plan.md.

Joseph Doherty 2ee84af1c0 feat(grpc): PSK-authenticate the site gRPC control plane; drop the vestigial management receptionist registration
Phase 0 of the ClusterClient→gRPC migration
(docs/plans/2026-07-22-clusterclient-to-grpc-plan.md). Standalone hardening: it
closes a gap that exists today and is a precondition for moving command/control
onto gRPC in later phases.

T0.1 — delete the ManagementActor ClusterClientReceptionist registration.
It was built for an out-of-cluster CLI that was never written: the shipped CLI
speaks HTTP Basic to /management, which asks the actor in-process through
ManagementActorHolder. Nothing in the repo ever sent to /user/management. The
actor still runs there; only the cross-boundary advertisement is gone. Six
documents claimed the CLI used ClusterClient — including the CLI's own README
"Architecture Notes" — and are corrected here rather than left to rot.

T0.2 — record, do not port, the dead integration-routing path.
IntegrationCallRequest is unwired at BOTH ends: RouteIntegrationCallAsync has
zero callers anywhere, and RegisterLocalHandler(Integration, …) appears only in
a test, so production always answers "Integration handler not available". It is
excluded from the gRPC contract (28 of 29 commands migrate) rather than
enshrined on an additive-only wire format, and deleting it during a
transport migration would mix a behavioural change into a change whose whole
value is that behaviour is identical. See
docs/known-issues/2026-07-22-integration-call-routing-is-dead-code.md.

T0.3 — preshared-key authentication on SiteStreamService.
The service shipped with no auth at all: plaintext h2c, no interceptor, so
anything that could reach a site node's :8083 could open a live data stream or
read audit rows back via PullAuditEvents/PullSiteCalls. ControlPlaneAuthInterceptor
now gates /sitestream.SiteStreamService/ — modeled on LocalDbSyncAuthInterceptor
(constant-time compare, fail-closed, PermissionDenied) but gating a SET of
service prefixes so phases 1A/1B add services rather than interceptors. LocalDb
sync keeps its own separate key: it authenticates the pair partner, not central,
and collapsing the two would make a site's central-facing key also admit writes
into its database.

Keys are per site (SB-GRPC-PSK-<siteId>), never fleet-wide, so a compromised
site yields only its own. Central attaches them through ControlPlaneCredentials,
which binds CallCredentials to the channel — covering unary and streaming
uniformly, and letting the key resolve asynchronously, which a client
interceptor could not do without blocking. All three central→site channel
creation sites go through it (SiteStreamGrpcClient and both audit pull invokers);
the pull invokers' channel caches are re-keyed by (site, endpoint) because
credentials are per-site and bound to the channel.

Two decisions beyond the plan:

  * StartupValidator now requires GrpcPsk on Site nodes. The plan specified only
    the runtime gate, but fail-closed with no boot check produces a node that
    joins, answers heartbeats and reports healthy while refusing every stream,
    audit pull and telemetry ingest — silent and total. Same reasoning as the
    existing inbound API-key pepper rule.

  * Added Communication:SitePsks as a central-side key map. The plan assumed
    central would read the store, seeded via a dev KEK; the docker rig
    deliberately boots with no master key, so store-only resolution would leave
    it unable to dial its own sites. The store stays primary — it is the only
    source that can serve a site added at runtime — with the map covering
    key-less hosts and one-off pins. Neither source falling back to
    "unauthenticated" is the invariant.

T0.4 — dev keys on both rigs and tests.
34 tests. The seven that matter most exercise a real in-process gRPC stack over
TestServer: the unit tests on either side of the wire would both stay green if
the halves disagreed, and gRPC refuses call credentials on a plaintext channel
by default — the UnsafeUseInsecureChannelCallCredentials opt-in is only provable
by making a real call. They confirm correct key passes on unary AND streaming,
wrong key and no-credentials both get PermissionDenied, and an unresolvable key
fails the call with nothing reaching the service.

OPERATIONAL: a site node upgraded to this build without a key will not boot.
That includes the gitignored deploy/wonder-app-vd03/ overlay.

Joseph Doherty 62cddcfa56 fix(audit): let an unresolvable cached row leave the drain queue
A cached-telemetry audit row whose OperationTracking snapshot could not be
resolved was skipped and left Pending. Nothing ever removed it, so the next
drain re-read it, failed identically, and logged again — forever.

The severity was worse than the original write-up said. The queue is read
oldest-first with a fixed BatchSize (default 256), so once a batch's worth of
permanently-unresolvable rows collected at the head, every drain re-read
exactly those rows and NEVER REACHED the newer rows behind them. That is a
permanent stall of the cached-telemetry path, not the "log flood, no data loss"
the issue was first filed as. Measured on the rig at ~2,800 warnings/minute,
surviving both a process restart and a container restart.

Fix: after a grace period (CachedTrackingGraceSeconds, default 300 s) the
operational half is abandoned and the row marked Forwarded. A row with no
CorrelationId is abandoned immediately — it can never resolve.

Marking Forwarded does not drop audit data. That state means "no longer owed by
the drain, still eligible for reconciliation", which is exactly this situation:
ReadPendingSinceAsync covers Forwarded as well as Pending and central dedups on
EventId, so the reconciliation pull still delivers the audit half. What is lost
is the operational (SiteCalls) half, which is unrecoverable anyway once the
tracking row is gone.

Three deliberate boundaries:
- Inside the grace window the row is still retried — a missing snapshot is
  normally a brief write race, and abandoning on the first failed lookup would
  discard the operational half of every cached call that lost that race.
- A tracking-store THROW never abandons, however old the row: a throw is a
  store fault (locked, corrupt, mid-restore), not a verdict about the row.
- Logging is per drain pass, not per row. Deferred rows dropped to Debug —
  being inside the grace window is normal operation, not a warning.

CachedDrain_OrphanRow_NoTrackingSnapshot_IsSkipped_DoesNotCrash was RETARGETED
rather than deleted: it pinned the defect ("skipped and stays Pending"), so its
orphan assertion is inverted and the half that still holds is kept verbatim.
Three tests added, including the starvation regression. Both "must NOT abandon"
tests carry a positive control on the read count, so they cannot pass by virtue
of a drain that never ran.

Verified non-vacuous: with abandonment disabled the two abandonment tests go

Joseph Doherty f2efeb37b7 refactor(sf,site): both stores take ILocalDb instead of a connection string
Tasks 5 and 6 of the Phase 2 plan, committed together because their test
fallout is entangled — several fixtures construct both stores.

StoreAndForwardStorage and SiteStorageService now take ILocalDb. Connections
come from ILocalDb.CreateConnection(), which hands out an already-open,
pragma-configured connection carrying the zb_hlc_next() UDF the capture triggers
call; a raw connection would lack the UDF and every write to a replicated table
would fail closed. Deleted with the connection strings: S&F's
EnsureDatabaseDirectoryExists and its per-open busy_timeout pragma, and the site
service's BusyTimeoutFloorSeconds normalization — LocalDb owns all of it now.

DI: AddSiteRuntime's string overload is gone (nothing left to supply), so the
Host calls the no-arg form. ScadaBridge:Database:SiteDbPath and
StoreAndForwardOptions.SqliteDbPath survive only as the migrator's source
locations in Tasks 8/9.

Two things the plan did not anticipate, both worth reading:

1. FOUND A REAL LATENT DEFECT, from Phase 1, now fixed. The plan assumed
   directory creation simply moved to LocalDb along with file ownership. It did
   not: the LocalDb library never creates the parent directory, and
   SqliteLocalDb opens the file eagerly in its constructor — so a missing
   directory is a hard boot failure ("SQLite Error 14: unable to open database
   file"), not a degraded start. The default site config points at the RELATIVE
   path ./data/site-localdb.db, so any site node without a pre-existing data/
   directory fails to boot. The docker rig escapes only because its volume mount
   happens to create /app/data — a coincidence that would have hidden this until
   a bare-metal or fresh deployment. This has been latent since Phase 1 made
   LocalDb:Path required; deleting S&F's EnsureDatabaseDirectoryExists here
   would have widened it. Re-established the guarantee at the layer that now
   owns the path (SiteLocalDbDirectory.Ensure, called before AddZbLocalDb) and
   pinned it with SiteLocalDbDirectoryTests. Non-vacuity is not assumed: two
   tests written against the wrong assumption failed with exactly this
   SQLite Error 14 before the fix existed.

2. Test fallout was ~7x the plan's estimate. The plan named "fixtures" in one
   project; the constructor change actually reaches 40 files across 7 test
   projects, and most used Mode=Memory;Cache=Shared — which LocalDb has no
   equivalent for, so every one had to move to a real temp file. Rather than
   copy the Phase 1 TestLocalDb fixture into 7 projects, added a shared
   tests/ZB.MOM.WW.ScadaBridge.TestSupport library (not a test project) so the
   WAL-sidecar cleanup and the "real, not stubbed" rationale live in one place.

Retargeted rather than deleted, in both directions: the S&F WAL test now asserts
against the LocalDb-backed store (WAL genuinely is LocalDb's job), while the
directory-creation test moved to Host.Tests (that guarantee is NOT LocalDb's).
SiteStorageServiceTests.Initialize_EnablesWalJournalMode got the same treatment.
DeploymentManagerMediumFindingsTests induced a persistence failure via an
unopenable path, which no longer reaches the assertion since the fixture now
throws first; it induces the same failure shape via an uninitialized store.

Verified: full solution build 0 warnings; SiteRuntime 532, Host 318,
AuditLog 355, ExternalSystemGateway 142, HealthMonitoring 97,
StoreAndForward 153 — 1597 passed, 0 failed.

Joseph Doherty 8652eab98e fix(localdb): root-cause the soak disk-I/O failure + ship the hardening follow-ups
The Phase 2 soak's "LocalDb fails under load" blocker is NOT a product defect.
Root cause: host-side (macOS) sqlite3 reads of the live, bind-mounted WAL
databases. POSIX advisory locks do not propagate across the virtiofs boundary,
so the host reader believes it is the only connection, checkpoints on close and
resets the WAL to 0 bytes under the container. The container's still-mapped
WAL index then references frames that no longer exist and every subsequent
statement fails SQLITE_IOERR_SHORT_READ (522) -> primary code 10, permanently
until the process reopens the database.

Reproduced on demand both on the rig (one sqlite3 SELECT reset a 4.6 MiB WAL and
produced the first error one second later) and in a minimal python:3.12-alpine
repro with no LocalDb or .NET involved. Refuted the converse: a freshly-reopened
node sustained the full soak load 10+ minutes with zero errors.

The original brief's isolation was confounded - both nodes had been poisoned by
the same sampling pass, and a poisoned standby looks healthy only because it
issues almost no statements. Corrections annotated in place.

Hardening shipped alongside:
- SqliteErrorCodes.Describe: log SQLite primary AND extended codes at the
  LocalDb-adjacent catch sites (the missing extended code is what made the
  original diagnosis so slow).
- SiteAuditTelemetryActor: stop touching ActorContext across an await
  (NotSupportedException), with regression coverage.
- infra/reseed.sh: apply the MSSQL init scripts explicitly. The official
  mssql/server image does not implement /docker-entrypoint-initdb.d, so a fresh
  volume hung the reseed forever; compose mounts annotated as informational.

Deliberately NOT done: detect-and-reopen self-heal in SqliteLocalDb. It defends
only against external interference, which is now prevented at the source, and
same-kernel production readers see the locks correctly.

Build 0 warnings; SiteAuditTelemetryActorTests 9/9, SiteEventLogging 70/70.

Joseph Doherty 0b5e9b44f6 feat(localdb): rewire OperationTrackingStore onto the consolidated site database
Task 4 of the LocalDb Phase 1 adoption plan.

OperationTrackingStore took IOptions<OperationTrackingOptions> and opened its own
SqliteConnection from ConnectionString. It now takes ILocalDb and gets every
connection - writer and the two ad-hoc reader paths - from CreateConnection().

This is not cosmetic. OperationTracking is a RegisterReplicated table, so its
capture triggers call zb_hlc_next(). That UDF is registered per connection by
ILocalDb and by nothing else, so a raw SqliteConnection would fail closed on
every write. Connections from CreateConnection() also arrive already open -
calling Open()/OpenAsync() on one throws - hence the removed OpenAsync calls on
the reader paths.

OperationTrackingOptions.ConnectionString is now vestigial for this store; the
database location is LocalDb:Path. The options class stays (retention settings)
and the config key stays bound for the site config DB.

InitializeSchema is kept but is now always a no-op in the host: onReady runs
while ILocalDb is being constructed, strictly before this constructor can
receive it. It remains so a directly-constructed store (tests, tooling) is
self-sufficient.

Tests: the store fixture moves off mode=memory&cache=shared onto a real ILocalDb
over a temp file. There is no in-memory mode - LocalDbOptions.Path is a
filesystem path - and testing through a raw in-memory connection would no longer
resemble the host. Verifier connections stay raw on purpose: this fixture never
registers the table, so no trigger fires and the UDF is never reached.

Three DI fixtures needed LocalDb:Path added, because resolving
IOperationTrackingStore now forces ILocalDb construction:
SiteCompositionRootTests, SiteAuditWiringTests, and the AuditLog
CombinedTelemetryHarness.

Verified: build 0 warnings; Host 294/294, SiteRuntime 530/530, AuditLog 354/354.

Joseph Doherty 2a0faeab6f fix(security): actually fix GHSA-2m69-gcr7-jv3q instead of suppressing it
The NuGetAuditSuppress in Directory.Packages.props was masking a LIVE high-severity
vulnerability, not documenting an accepted one. Only the Host project resolved a
patched SQLitePCLRaw.lib.e_sqlite3 2.1.12 (transitively, via ZB.MOM.WW.Auth.ApiKeys).
Every other SQLite consumer - AuditLog, SiteRuntime, StoreAndForward, SiteEventLogging
and 11 test projects - still resolved the vulnerable 2.1.11.

The suppression's rationale was factually wrong: it claimed 'the only patched native
lib is the SQLitePCLRaw 3.x line'. 2.1.12 patches this advisory within the 2.1.x line,
so the feared risky force-override of the whole family to 3.x is unnecessary.

Fix: explicit PackageReference to the patched 2.1.12 in each SQLite-consuming project,
plus the central PackageVersion row. Suppression removed, so the advisory is audited
again rather than silenced.

Rejected alternative: CentralPackageTransitivePinningEnabled. It clears the advisory in
one line but makes every central version a ceiling for transitive resolution, demanding
bumps to Google.Protobuf, Grpc.Net.Client, Microsoft.Data.SqlClient and Newtonsoft.Json.
That is a gRPC/data-access change to a production SCADA platform and deserves its own
reviewed commit.

Verified: forced full-solution restore reports no NU1903 (only the pre-existing,
unrelated AngleSharp NU1902 in CentralUI.Tests); all four previously-vulnerable src
projects now resolve 2.1.12; 55 projects build 0 warnings / 0 errors; 1108 tests pass
across the SQLite layer (AuditLog 354, ConfigurationDatabase 357, Security 181,
StoreAndForward 152, SiteEventLogging 64).

Joseph Doherty 9110a4eb01 fix(auditlog,health): harden hosted-service shutdown against disposed CTS
The host does not guarantee IHostedService.StopAsync is driven before the DI
container is disposed — WebApplicationFactory's teardown reaches Dispose first
— so cancelling the internal CTS from StopAsync threw ObjectDisposedException
and aborted the host's whole shutdown sequence. Four services shared the same
copy-pasted lifecycle and the same two races: StopAsync cancelling an already-
disposed CTS, and StartAsync reading _cts.Token lazily inside the Task.Run
lambda, which faults the loop task the host awaits when Dispose wins that race.

Each service now captures the token on the caller's thread, tolerates a
disposed CTS, and cancels-before-disposing so the loop is always signalled and
its pending Task.Delay sees a cancelled token rather than a dead source.
SiteAuditBacklogReporter also gains the outer OperationCanceledException guard
its sibling SiteAuditRetentionService already carried (arch-review 04 R2, R7),
without which a shutdown landing mid-probe threw TaskCanceledException out of
Host.StopAsync.

Surfaced while verifying the Gitea #15 test-harness fix: in Host.Tests the
aborted teardown skipped the fixture's env-var restore, contaminating every
later test in the run.

Joseph Doherty bfd2cc7e6f fix(audit-log): reconciliation pulls fail over to site NodeB when NodeA is unreachable
SiteEntry gains an additive FallbackGrpcEndpoint; SiteEnumerator now picks NodeA
else NodeB as primary (a NodeB-only site is no longer skipped) and carries a
distinct NodeB as the fallback. Both GrpcPull{AuditEvents,SiteCalls} clients dial
the primary first and, on a transport fault (Unavailable/DeadlineExceeded/
Cancelled/HttpRequestException/SocketException/OperationCanceled), fail over to
NodeB once before collapsing to empty — a mapping/unexpected fault does NOT fail
over. Keeps the reconciliation loss-recovery net available during a NodeA outage.

abandon threshold, the reconciliation actor now writes ONE synthetic
ReconciliationAbandoned row (new AuditKind) alongside the Critical log line —
fresh EventId, Status=Failed, channel preserved from the lost row, Extra carrying
the abandoned EventId + source site + final error — via the same
ScadaBridgeAuditEventFactory the ingest path uses. Best-effort in its own
try/catch so it never blocks the cursor twice. The permanent loss is now
queryable in the Audit Log, not only in a rotating log file.

sharing one exact UpdatedAtUtc now drains via the id tiebreak instead of pinning
forever. A legacy site that ignores after_id is latched + published as
SiteCallReconciliationPinnedChanged on the EventStream (transition-only),
replacing the prior silent log line.

Fixes the 8 findings from the 2026-06-24 re-review (commit c42bb485), with a
regression test per Medium finding:

- DataConnectionLayer-029 (Med): HandleAlarmSubscribeCompleted now mirrors the
  tag-path re-check — if a feed is already stored for the source, release the
  redundant just-created subscription instead of overwriting + leaking the first
  one (the double-subscribe window DCL-023 reopened). +regression test.
- InboundAPI-031 (Med): remove WaitForAttribute's local 5s grace backstop (tighter
  than the CommunicationService Ask's timeout+IntegrationTimeout round-trip budget,
  so a slow-but-valid timed-out 'false' got cancelled into a 500). Link only the
  client-abort + explicit caller tokens; the lower layer owns the backstop. +test.
- SiteRuntime-032 (Med): derive the deployed count from an authoritative set of
  deployed config names (HashSet) instead of a map-presence-gated int, so deleting
  a DISABLED instance decrements correctly (SiteRuntime-029's gate leaked it).
  +deploy->disable->delete regression test.
- StoreAndForward-028 (Med): reset _bufferedCount in StopAsync alongside the
  register-guard so a same-instance Stop->Start re-seeds from a clean base (no ~2N
  gauge double-count). +restart regression test.
- AuditLog-017 (Low): test the OnIngestAsync scope-resolution guard (actor survives,
  replies empty, counts the failure) — no longer unpinned.
- CentralUI-037 / ScriptAnalysis-009 / SiteRuntime-033 (Low): doc-comment + spec
  fixes (Database-throws in the inbound sandbox; baseReferences param wording;
  native-alarm cap return-to-normal + per-condition NativeAlarmDropped eviction).

1. Request headers in Extra JSON (AuditWriteMiddleware): adds a `requestHeaders`
   object to the existing Extra JSON alongside remoteIp/userAgent; headers whose
   names appear in AuditLogOptions.HeaderRedactList (Authorization, X-Api-Key,
   Cookie, Set-Cookie by default) are replaced with "<redacted>" using
   OrdinalIgnoreCase matching — same policy as ScadaBridgeAuditRedactor.

2. AuditInboundCeilingHits counter: new IAuditInboundCeilingHitsCounter interface

   the NoOp default, AddAuditLogCentralMaintenance forwards to the snapshot;
   AuditWriteMiddleware accepts the counter as an optional ctor arg and increments
   it once per request where either the request or response body hit the cap.

3. Per-method SkipBodyCapture opt-out: adds SkipBodyCapture bool to
   PerTargetRedactionOverride; AuditWriteMiddleware consults the per-target
   override map at the start of InvokeAsync (before EnableBuffering) and, when
   set, skips body read + capture entirely — the audit row still emits with
   headers/metadata but null RequestSummary/ResponseSummary; truncation flags
   are also cleared so the ceiling-hits counter is not bumped for opted-out methods.

Joseph Doherty 963e3427da feat(sitecallaudit): PullSiteCalls reconciliation plumbing (store read + RPC + site handler + central client)
Site Call Audit (#22): build the documented periodic reconciliation PULL
self-heal path for the eventually-consistent central SiteCalls mirror, as a
dedicated PullSiteCalls gRPC RPC kept separate from the audit pull. This is the
pull PLUMBING only; the central reconciliation tick is a separate follow-up.

- IOperationTrackingStore.ReadChangedSinceAsync(sinceUtc, batchSize): inclusive
  UpdatedAtUtc cursor, oldest-first, batch-capped; SQLite impl projects tracking
  rows onto SiteCallOperational (Kind->Channel, TargetSummary->Target, SourceSite
  left empty - the store has no site-id column).
- sitestream.proto: rpc PullSiteCalls + PullSiteCallsRequest/Response, mirroring
  PullAuditEvents; regenerated checked-in SiteStreamGrpc/*.cs.
- SiteCallDtoMapper.ToDto(SiteCallOperational): inverse of FromDto for the handler.
- SiteStreamGrpcServer.PullSiteCalls handler + SetOperationTrackingStore seam;
  Host wires the seam alongside SetSiteAuditQueue (site roles only).
- Central IPullSiteCallsClient + GrpcPullSiteCallsClient (home: AuditLog/Central to
  reuse ISiteEnumerator; SiteCallAudit does not reference AuditLog). Re-stamps
  SourceSite from the dialed siteId; no-throw on tolerable transport faults;
  SpecifyKind (not ToUniversalTime) cursor handling. Central-only DI registration.

Tests: ReadChangedSinceAsync (4), PullSiteCalls handler (6), GrpcPullSiteCallsClient
(8). Full solution build 0 warnings/0 errors (TreatWarningsAsErrors).
